多点执业受到行政制度干扰
2009年12月和今年1月，广东省卫生厅先后印发关于医师多点执业的管理办法。廖新波表示，广东率先在全国试行医师多点执业，一是为了扶持下级医院，实现医疗资源合法有效流动、优化配置；另一个目的是改变过去医生只能单点执业的情况，令医生的“走穴”阳光化。
廖新波透露，从2010年1月试点医师多点执业至今，整个广东省16万多名注册医师，仅有3800多人申请。从全国来看，此后试点的海南、云南、四川、北京、江苏等地也和广东一样，并未出现预期的医生自由流动大爆发，效果不尽如人意。分析其原因，廖新波认为，多点执业受到行政制度的干扰。例如人事制度里，超编是一种逆政策的行为。“最重要的，是受传统的‘单位人’观念束缚。医生本身希望有一个寄身的地方，一般都希望能进入大医院。一旦进去，更多的是不愿意走动。而且大医院的任务很重，他们也没有能力走动。”廖新波说。
“解放医生”进行市场化流动
如何破解医生多点执业的困局？廖新波认为，让医生流动起来，还是需要根据市场化的原则，医院和医生之间签订服务条约。医院为了留住人才，竞相为优秀的医生提供富有竞争力的薪水和平台；医生亦需为定点执业的所有医院提供优质的服务。一旦出了医疗事故，由当事医生和当事医院负责，由法律来裁定如何赔偿，与第一执业单位无关。“只有解放医生，才能实现真正核心的医疗资源市场化流动，‘看病难’亦可随着医生的自由流动迎刃而解。”廖新波说。
