11月20日零时19分，宁波建工股份有限公司承建的宁波工业供水有限公司工业供水管道青峙支线施工现场发生事故，造成五人死亡，一人受伤。伤者目前生命体征稳定，正在医院接受治疗。
据初步调查，该工程名为工业供水青峙支线管道项目，建设中的青峙支线起点为通途路北延线，终点为滨海快速道，全长约6.93公里，事故地点为6号工作井，事故发生时施工人员正在工作井内进行顶管作业。
事故发生后，北仑区立即启动安全生产事故应急救援预案，区委、区政府和市、区安监部门等主要领导第一时间赶到事故现场，指挥现场救援和善后处理工作。目前，宁波市事故调查组已进驻当地开展事故调查工作。当地公安机关已对1名事故项目负责人、两名现场施工管理人员进行控制。具体事故原因正在进一步调查之中。
[ 探访现场 ] 遇难者嘴巴微张面部发黑，身上并无伤痕
昨日早上10点多，事发地已拉起警戒线，记者试图进入却遭到民警阻拦。
站在高处远远望去，这是一个圆柱形的在建工地，没有塌方迹象。附近一居民说，凌晨一点左右，看到有消防人员戴着空气呼吸器进入到事故现场，将一个个尸体拖出来。
在事故现场门口，记者遇到了七八位死者的家属和亲戚。朱先生就是其中一名死者的女婿，他是较早得知事故消息的人。“凌晨五点半左右接到包工头何国卫的电话，他说，出大事了，我一听慌了，赶紧打电话挨个通知老乡和家属。”朱先生说。
据朱先生说，遇难的六名工人中仅有一名幸存者被送往镇海龙赛医院治疗，其余五名当场死亡。随后，记者在北仑殡仪馆里见到了五名死者，他们每个人死状几乎如出一辙：面部发黑，头部以及脸部位都是泥巴，身上并无伤痕，嘴巴微微张开。
现场一名死者亲属严正发说，他们都是来自江西鄱阳县三庙前乡栎树村车马湖自然村。其中死者严旺田和严稳和是父子；死者严旺田和严青田是亲兄弟；死者严稳和、严稳发是堂兄弟；严小伍和严稳和、严稳发都是好朋友。
严正发介绍，他们的村子里有四百多户人家，其中就有100余名青壮年外出打工做土方施工，当地称“顶管”。
“我们村子里有做‘顶管’的传统，村民在外打工主要靠这个挣钱。”严正发说，出事的是一个班组，领班的是52岁的严旺田，他做这行几十年了从没出过事，没想到这次出了这么大的事。

昨日中午12点多，记者辗转找到唯一一名幸存者严红国，他今年30岁。此时，他正在高压氧治疗室输氧，初步诊断结果是气体中毒。医生表示，严红国已经脱离生命危险，生命体征平稳，但肺部有损，还需住院观察。下午1点多，严红国从高压氧治疗室出来后，被转入呼吸科普通病房。严红国回忆了事发经过：
吃过晚饭后，大概七八点钟去施工，在地底下顶了大概还有两米多的样子，顶到了一个水管一样的东西，里面是钢管，外面是软皮管。可能是有人去拉了一下软皮管，里面就有点漏气了，气味有，但是不大，当时我们谁也没在意，后来，我们都晕倒了，等我醒来就发现，其他人已经躺地上了，我整个人迷迷糊糊昏昏沉沉的，自己好像被东西压着，喘不过气。后来，我就自己爬上来，扶着走出去。
严红国告诉记者，他干这个活儿也是老乡介绍过来的，才干了22天，一天100多块工钱。下井前并没有戴防护罩，只是戴了安全帽，施工作业前也没有任何单位要求他们岗前培训，也没有任何安全生产的提醒和警示。
做这行的都是老乡带老乡 不签协议不培训全凭经验
●解读“顶管”之乡
死者亲属严正发说，尽管做土方这行危险性高，但村里还是有很多青壮年到大城市里来揽活，为的就是一个月能挣三四千块钱的工钱。以往他们也常听说施工过程中塌方的情况，但是气体中毒的情况还是第一次听说。
10多年前，严正发也从事过类似工作，但是由于这项工作危险性极高，如今就没再做这一行了。关于这次事故，他凭着自己多年的经验给了这样的判断：“一般情况下，做这种活儿都是先打一口井，约八九米深，然后再根据图纸去挖。按照正规流程，这家企业应该事先探测好前方地形及管道情况，工人施工作业时遇到比如沼气管道、煤气管道等，施工方要提醒工人避开，偏角度施工。”严正发说。
“像宁波起码半数以上铺设管道的土方工程，都是我们鄱阳县的工人在做的。像我叔叔严旺田(死者)就是干了十五年的老师傅了，他就从来没有过任何上岗培训，去任何一家公司和工地作业，从来不会签协议，全凭经验操作。”严正发说，工人们都是老乡带老乡一起来宁波干活，有一桩活干一桩，而包工头承包的一些市政工程中，往往有一部分还是多番转手包来的工程。

年底事故频发往往有迹可循，比如一些工地赶工期，一些单位为了考核目标的完成，建设监管、工人施工都易思想麻痹等等。可是多人殒命，毕竟是不忍看到的惨烈现实，令人非常痛心。
仔细梳理分析每一起事故，都可以发现若干问题，都是多个安全漏洞的叠加，并非无法避免。比如施工各方安全意识淡薄、工地管理混乱、源头监管薄弱等等。城市正处在大建设时期，到处都有工地，对施工安全的管理，必须尤为注意。出了事之后，再亡羊补牢，总是令人遗憾的。死一个人，一个家庭就破碎了，生命无法重来，警示必须把“生命第一”的理念落实到施工管理的全过程。而每一起事故的发生，也告诉我们，要想达到高起点、高层次的平安宁波建设目标，还有太多的管理漏洞要补上，还有太多的细致工作要做。
事故提醒我们，建筑施工的监督检查不能流于形式，工人的安全教育不能成为空白，每一项规章制度不能成为虚设；事故还提醒我们，对工地的管理应提高到更高的水平，地面及地下的复杂情况，必须有专业技术人员现场指导，不能靠工人自己凭感觉、凭经验“摸爬滚打”；事故更提醒我们，重生产、轻安全，重形式、轻管理的粗放模式必须得到纠正，否则还会有事故发生，还会有人员伤亡。
