2013年11月至12月，我省汉寿县、衡山县、常宁市先后报告了3例接种乙肝疫苗后疑似预防接种异常反应病例。经过现场调查，我省各级疾病预防控制中心、预防接种单位疫苗储藏、运输符合规范要求，3例病例疫苗接种的单位、接种人员具有相应资质，接种人员接种操作规范。
省疾控中心副主任高立冬介绍，2014年1月2日和8日，省卫生厅分两次组织省级预防接种异常反应专家组综合临床病历资料、现场调查资料、疫苗检测结果和2例死亡病例的尸检结果，分别对3例病例进行了综合诊断。
衡山县病例是一个30天大的婴儿，接种疫苗前后可能受凉，当时临床表现为纳差(即进食质量不好)，精神萎靡、发绀，符合婴儿肺炎表现；尸检结果为支气管肺炎合并肺出血，符合重症肺炎改变。气道内有较多灰白色混浊液，不能排除返流误吸加速患儿死亡。因此，诊断该病例为急性支气管肺炎(重症)。
常宁市病例接种疫苗后至入睡前10小时无异常，入睡后至死亡情况不明。尸检发现双手指甲青紫，无喉头水肿；重要器官淤血、水肿，有淋巴细胞、单核细胞浸润；尸检未见过敏反应的病理形态学证据。因此，诊断该病例捂被综合征(寒冷季节家长给小儿捂被子保暖过度所致)可能性大，不排除病毒感染。
汉寿县病例接种乙肝疫苗和注射维生素K1，约2小时后出现过敏性休克，过敏性休克可能性大，但维生素K1、乙肝疫苗均有引起过敏性休克的可能，过敏原不确定。患儿在太子庙镇卫生院经胸外心脏按压等急救措施抢救约1分钟后喷出15-20毫升母乳凝块，不能排除窒息的原因。
高立冬表示，偶合症不属于预防接种异常反应，但最容易造成误解。偶合症是指受种者正处于某种疾病的潜伏期，或者存在尚未发现的基础疾病，接种后巧合发病(复发或加重)，因此偶合症的发生与疫苗本身无关。疫苗接种率越高、品种越多，偶合症的发生越多。
预防接种是一项公共卫生措施，在保护绝大多数人群健康的同时，极个别人承担了发生异常反应的风险。为了充分考虑到受种者的权益，《湖南省预防接种异常反应补偿办法》规定，对因异常反应引起的严重损害者给予一次性补偿。据介绍，我省将按照分级补偿标准，对汉寿病例给予相应补偿。
