  南方日报讯 (记者/曹斯 通讯员/简文杨)近日一则“胃爆炸”的传闻在网络上疯传,称一名58岁的患者由于大量进食导致“胃爆炸”,术中还着火,最后不得不全胃切除。昨日,中山大学附属第六医院大外科主任、胃肠外科主任林锋教授接受采访时表示,“胃爆炸”说法言过其实,“会着火”更不科学,如果该病例施行了“胃切除”的手术,应基于其他病理原因。


  胃壁穿孔不能称“爆炸”


  这条“胃爆炸”的传闻来自著名微博达人“@营养师顾中一”的爆料。这位北京友谊医院的营养师在微博上转发了一个病例:患者女性,58岁,进食大量食物及啤酒,餐后突发腹痛腹胀1小时来院急诊,患者腹胀如鼓,全腹压痛。腹部CT显示,胃扩张。手术时,医生发现腹腔内高压可燃气体喷出,灭火后手术继续。腹腔内大量黑色浑浊液体混有食物残渣,胃壁破损严重,胃小弯贲门至幽门全程炸裂。又是“胃爆炸”,又是“起火”,这场手术真是让人紧张。最后的结果也是十分令人遗憾,由于患者病情太重,不得不全胃切除。


  在林锋看来,“爆炸”的说法太夸张,暴饮暴食确实有可能引发急性胃扩张,也就是俗称的“胃瘫”,“这是最严重的一种情况,会使得胃部失去动力,高压扩张,胃壁受挤压,粘膜破损出血最后穿孔。但这种现象不能称为爆炸,可表述为像气球破了个洞,而洞有大有小之分而已。”


  所谓“可燃气体”不可能


  至于微博所提及的“高压可燃气体”,林锋也认为这不可能,“一般情况下人体胃部内70%的气体来自呼吸或者咀嚼等过程中吸入的空气,而当前主流饮料和啤酒中较多含有的是二氧化碳,是不可燃甚至是阻燃的气体,远远达不到被点燃的条件。就算是人进食的有机物质有机会在胃里发酵,产生类似沼气类的可燃物体,也需要较长的时间,不可能在短时间内形成。”


  林锋表示,人体有非常完善的自我保障机制,一旦胃部潴留过量时,会自发的出现呕吐、打嗝等反应,强制减压,这不为人的意识所控制。


  急性胃扩张大多不需手术


  至于胃切除手术,作为中国抗癌协会胃癌专业委员会外科常委兼外科组组长,林锋表示只有在胃发生病理性质转变时,即肿瘤、血运障碍以及广泛胃出血等的情况下,才考虑此种手术。


  他介绍说,临床中,较多出现“胃瘫”的主要是一些颅神经损伤、脑中风、休克、感染或是大型手术后的患者。至于暴饮暴食引起的急性胃扩张,则大部分是因为患者年龄比较大,或是有糖尿病史,或是有胃溃疡、慢性萎缩性胃炎病史等。


  林锋表示,急性胃扩张的发病是一个持续的过程,一般伴有饱胀的感觉和呕吐现象。“洗胃”、插胃管、减压为常用的治疗措施,大多数情况下不需手术,“当然,急性胃扩张导致粘膜受损出血,小的话可以在内镜下进行治疗,而一旦出血量较大或有穿孔则会考虑手术,如果置之不理必然会比较危险。”


  林锋认为,在没有强调全胃切除的缘由是基于其他病理原因的基础上,很容易给人造成胃穿孔就要胃切除的假象,易误导大众。不过他也强调,虽然“爆炸”的说法夸张,但大吃大喝刺激肠胃是没商量的,除急性胃扩张、胃穿孔外,严重时还会引发急性胰腺炎,甚至危及生命,因此假日吃喝还是要悠着点。
