明确诊断是合理治疗的关键，但要判别一个所谓的“双性人”到底是男孩还是女孩，绝非易事。“不仅要弄清病因，了解性腺功能状况，还要结合孩子心理进行综合评估”，据巩纯秀教授介绍，在同样的检查结果面前，有经验的医生就像一名出色的侦探，更能在错 综复杂的“案情”中迅速准确地找到答案。
法制晚报记者了解到，对于诊断明确的病例，专家建议，内分泌评估之后，结合家长的性别抚养意愿，可联系泌尿外科进行手术;暂时无法明确的孩子，则采取暂定一个性别，待青春期孩子发育情况更明晰之后再做诊 断;如果孩子已经长大成人，依然难以给予医学判断，就让孩子自己进行性别选择。
“即使在西方发达国家，目前，也是差不多一半的病例无法明确做出精准的病因诊断”，巩教授表示，这既受限于医学技术，更来自于社会观念。“传统观点总觉得性发育异常是难以启齿的，而此病又与遗传密切相关，光看孩子，看不到亲属，很难找准病因、也就无从明确诊断了”。
正因为此，2006年，欧洲儿科内分泌协会特别提出，要用性发育异常来代替带有性歧视的术语。 事实上，很多患有性发育疾病的孩子，除性器官外表和性腺功能异常外，其他脏器功能都很健康。巩教授强调，对于这些孩子，医学予以合理的治疗，社会给予足够 的尊重，他们一样能健康成长。
