8月11日下午，湖北当阳市马店矸石发电有限责任公司（简称“马店发电公司”）热电项目在建调试过程中，发生高压蒸汽管道爆炸事故，致21死5伤。
记者从国家安监总局了解到，事故主要原因系管道流量计阀门焊缝裂开，大量高温高压蒸汽外溢，导致主控室玻璃破裂，造成主控室人员严重伤亡。
当阳市相关负责人表示，目前涉事企业设备已全部关停，公司法人及相关人员共6人被警方控制。
事发时现场近600℃
管道爆裂处位于马店发电公司厂址中部一处千余平米的厂房内。昨日上午11时，该厂房3层一面积约100平米的房间内，仍处于高温炙热状态。
现场工作人员介绍，事发时，房内的一条蒸汽管道在调试过程中发生破裂，现场温度近600℃，导致作业的21人不幸遇难。
在事发核心区域，一条直径约1米的银白色高压蒸汽管道出现爆裂，只剩下里层一段较细的深色管道，管道外膜和填充物完全损毁。地上随处可见管道外膜的碎片，能承受重压的金属外膜也被冲击变形。
紧邻事发地的是个操作间，由一面大窗户相隔，受爆裂冲击只剩下框架。房间天花板被炸毁，随处可见玻璃碎片和扭曲的金属条，已散架的桌椅东倒西歪，一片狼藉。
涉事企业法人被控制
新京报记者从当阳市委宣传部了解到，事发后，涉事公司所有设备已关停，周边企业也一并关停，中国公安部门对现场进行严格管控和警戒，并做好事故现场保护工作。国家安监总局工作组昨日上午已到当阳，现场指导事故处理工作。
当阳市常务副市长李小军称，马店矸石发电有限责任公司法人已被控制，共6名相关人员正在当阳市公安机关接受调查。

阀门焊缝裂开，蒸汽外溢
马店发电公司员工介绍，爆裂点位于一栋五层楼厂房，长80多米，宽30多米，各层都布置了形如长城烽火台状的管道。一楼有两台大型发电机，屋后有三座大锅炉。
蒸汽外泄点位于三楼的中控台，长约20米，中控台与管道之间为双层玻璃隔开。管道直径426mm，外层包有300mm左右厚度的白色保温棉。“一靠近就全身发热，站两分钟就满头大汗，不到10分钟就要到外面去喘口气。”
在工人们看来，哪怕一个小孔蒸汽外泄也能致命，“像针一样，高温可以把身体刺穿。”
此次高压蒸汽管道爆裂的原因何在？
据国家安监总局消息，经初步调查分析，事故主要原因是，2号锅炉蒸汽出口处主管道流量计阀门焊缝裂开，大量高温高压蒸汽外溢，致主控室玻璃破裂，造成主控室人员严重伤亡。事故详细原因正在进一步调查中。
国家电网中国电力科学研究院副总工程师蔡国雄介绍，出现事故的蒸汽管道，是火电厂的标配。操作中，一般会将煤炭和煤矸石混合燃烧，待锅炉燃烧结 束后产生蒸汽，再用蒸汽推动蒸汽轮机。其中，锅炉和蒸汽轮机间有一段距离，此次发生事故的就是这一部分输送高温高压蒸汽的管道。
他说，管道焊接位置的强度弱于其他部分，如果存在焊接质量不好，或在长期运行中，焊缝处逐渐发生开裂均可能出现问题。因此，平日里，对焊接处进 行检测尤为重要。一般情况下，在焊接后，还要对焊缝质量进行超声波鉴定，确保每条焊缝的安全。“一旦瞬间蒸汽流量变大，锅炉压力和温度增高，就可能导致焊 缝开裂”。
“蒸汽本身没有毒性，但温度和压力很高，厂房是封闭的，冲击波会把建筑物、窗户打碎，人一下子会被推倒，造成烫伤等情况。”他说，主控室日常并 不需要太多运行人员，但如果当时正在调试，有可能安装和调试人员均在现场，数百摄氏度的蒸汽确实可能会造成较大的人员伤亡情况。
焦点2 死伤者身份
均为公司员工，最小22岁
当阳市人民医院重症监护室门口，黄华林的姐夫姚平彻夜未眠。他介绍，48岁的黄华林是发电厂的“元老”，10多年前在马店村建厂时就第一批入厂，还被派到江西学习管理仪器。
事发当天，黄华林是白班，还有2个多小时就要下班了，然而，事故发生，他的烧伤面积达80%。“全身都是黑的，眼睛睁不开，手上起了泡，腿上缠着纱布，话也说不出来了”。
24岁的罗文是这次事故5名伤者中最年轻、也是烧伤最严重的，达90%。“医生说目前仍有意识，喊他名字能做出反应，眼能睁开，就是肿得厉害。”其家属介绍。
罗文只在发电厂上班两个月，负责照看仪器。此前在化肥厂打过工。据家属介绍，罗文距离爆炸的管道有点距离，所以只是受了伤。
当阳市常务副市长李小军说，事故发生后，宜昌市和当阳市均启动应急预案，当地公安、卫生、医务部门第一时间赶赴现场，消防多次进入事发现场，共发现遇难遗体21具，已全部送往殡仪馆。
新京报记者获得的一份此次安全事故受害者名单显示，21名死者和5名伤者均为当阳籍，且为马店发电公司职工，均办理了“五险一金”，来自当阳市 坝陵办事处、玉阳办事处、王店镇、两河镇等多个乡镇。遇难者中年纪最小者22周岁，目前正通过比对DNA，对遇难者身份进一步核实。
当阳市卫生计生局局长龚煊介绍，事故发生后，5名伤者被送到当阳市人民医院，其中3人伤势较重，全身90%烧伤，一度出现昏迷休克情况，经武汉 同济医院、武汉市三医院、宜昌市医院专家会诊后，生命体征基本稳定。目前，相关部门正在调动直升机，将4名伤员转至武汉三医院接受进一步治疗。
焦点3 电厂安监
排放超标曾被查，发生过泄漏
附近村民介绍，该公司厂址原是座山坡，去年10月从马店村迁移过来，把山坡推平建造了厂房。
工商资料显示，该公司于2002年成立，注册资本1亿元，经营范围包括火力发电，炉渣、炉灰销售，煤炭购销，乙醇汽油、煤油、柴油、润滑油零售等。
据国家安监总局消息，该公司由华强化工集团（占67%股份）和马店矸石发电厂（占33%股份）共同出资组建。其热电联产项目由湖北省中国国家发展改革委员会于2012年12月批准，去年1月开工建设。主要设备安装今年5月完成，具备调试条件，从6月中旬开始进行设备调试至今。
湖北省中国国家发展改革委员会官网资料显示，该项目总装机25MW，建设内容为4台220t/h高温高压循环流化床锅炉（三开一备），配1台12.5MW背压式 汽轮发电机组和一台12.5MW抽汽背压式汽轮发电机组。去年，省中国国家发展改革委员会下发通知，同意该项目所发电量为华强化工集团自发自用，余量上网。
新京报记者在厂区一标示牌上发现，该项目总投资达8亿元，占地550亩，“建成后可解决社会就业500余人，新增销售收入7.6亿元，新增利税1亿元，且已于今年5月投产”。
今年8月，《三峡晚报》报道，该项目全部投产后，实现“以热定电、能量梯级利用”，为工业园区内企业实现集中供热供气，同时也解决了园区内因煤矸石和造气炉渣堆存占地和环境污染问题。
但该公司曾出现过安全环保隐患。湖北省环保厅官网显示，该公司2012年提交的环评报告中，曾指出其主要环境风险，包括油罐泄漏后遇火源的火灾、爆炸事故；各类储罐泄漏事故。去年，该公司因出现废气排放超标现象，被当阳市环保局查处。
公司多名员工表示，该项目存在违规情况。一位调度员称，部分蒸汽通过管道运送到华强化工公司，今年曾发生一次泄漏事件，所幸没有人员伤亡。
此外，一名包工头说，该公司管道修补的情况较常见，“如果有泄漏，先剥掉保温棉焊接好，再重新装上，这期间机器还在运转”。
