此后数年间，该案历经七审三判，均因“事实不清，证据不足”被上级法院发回重审。2004年5月，平顶山中院在证据不足、争议较大的情况下，为了避免被害人家属上访，与其约定尽量判李怀亮死刑，该案也因此被称“死刑保证书案”。
在社会各界的关注下，2013年4月25日，平顶山中院再次开庭审理该案，认定李怀亮犯故意杀人罪证据不足，判决宣告李怀亮无罪，不承担民事赔偿责任。被关押了12年的李怀亮当庭释放。
判决宣告后，李怀亮提出国家赔偿申请共计380.1247万元，包含限制人身自由赔偿金及精神损害抚慰金、医疗费、护理费、误工费、房屋修理费、异地生活安置费、家人12年的奔波费等十四项赔偿、补偿请求。
平顶山中院依法进行审查后认为，李怀亮因涉嫌故意杀人被限制人身自由，先后被判处死刑、死缓，后宣告无罪，实际被限制人身自由4282天。按照最高人民法院公布的2013年国家赔偿标准，侵犯公民人身自由权每日赔偿182.35元，赔偿其被限制人身自由赔偿金78.08227万元。
根据李怀亮被羁押时间、工作生活所受到的影响、结合当地经济社会发展状况及平均生活水平，酌定精神损害抚慰金20万元。对李怀亮提出的其他请求，因不属国家赔偿法规定的赔偿范围，不予支持。
