省计生委已向省政府递交《关于启动实施政策的请示》，方案须报国家备案后提交省人大常委会决定

12月17日，记者从省计生委了解到，湖南已启动“单独两孩(一方是独生子女的夫妇可生育两个孩子)”实施方案制定程序，待省人大常委会作出决定后，将尽快组织实施。
政策“很快”实施，取消过渡方案
11月12日下午闭幕的十八届三中全会，正式审议通过了《中共中央关于全面深化改革若干重大问题的决定》。决定关于“启动实施一方是独生子女的夫妇可生育两个孩子的政策”，使得一直以来甚嚣尘上的“单独两孩”政策终于尘埃落定。
当前，最受关注的问题莫过于“单独”夫妇什么时候可以生第二胎？一位计生系统人士透露，具体时间还没有定，但是会“很快”。据称，原本湖南准备在出台“单独两孩”政策之前，制定一个过渡方案。最后由于考虑到政策很快就会正式实施，便取消了出台过渡方案的计划。
此前，国家卫生计生委计划生育基层指导司负责人在接受采访时曾表示，启动实施“单独两孩”政策，全国不设统一的时间表，将由各省(区、市)根据实际情况，确定具体时间，通过省级人民代表大会或其常委会修订地方条例或作出规定，稳妥、扎实、有序、依法组织实施。当“单独”夫妇户籍所在的省份修订了人口与计划生育条例或人大常委会作出了专门规定，允许单独夫妇可生育两个孩子后，就可按程序申请再生育。
将指导符合政策夫妇做好孕前优生
省计生委表示，中央提出启动实施“单独两孩”政策之后，省计生委及时学习领会中央精神，并按照要求有序开展了相关调查测算、风险评估等准备工作。目前，已向省政府递交《关于启动实施政策的请示》，并向省人大相关专门委员会和省法制办汇报和衔接有关法律程序工作，待“实施方案”报国家备案后，再提交省人大常委会作出决定，依法、稳妥、扎实、有序地尽快组织实施。
同时，一位接近省计生委的人士透露，整个实施方案非常人性化，最大的亮点就在于实施时间。此外，方案对于公众关心的一些细节，诸如“已经怀孕了该怎么办”等系列问题，也做出了规定。
此外，为了避免生育潜能的集中释放，省计生委相关负责人还表示，将会同有关部门建立健全孕产期保健、住院分娩、婚姻、户籍等信息共享机制，加强入户采集、办事采集信息，准确把握出生人口动态。在有效的人口监测基础上，及时作出预报预警和处置。省计生委将根据“十二五”人口发展规划、近年出生人口变动情况以及“单独两孩”政策启动实施情况，编制年度人口计划，加强宏观调控。同时，人口计生技术服务人员还将对有生育意愿且符合“单独两孩”政策的夫妇进行指导，帮助他们做好孕前优生准备工作。
再生育问与答
不同避孕方式，再妊娠等待时间不同
问：多少岁算高龄产妇？如果到了那个年纪，生第二胎要做些什么准备？
游一平(湖南省妇幼保健院大产科主任)：25至30岁，是女性最佳生育年龄，医学上将35岁以上的产妇称为“高龄产妇”，属于高危妊娠范畴。高龄妈妈在孕前3个月就要服用小剂量叶酸，可以减少胎儿神经管畸形的发生。此外，准爸爸也有检查要做，包括血、尿常规，肝肾功能和精液检查等，所以准爸爸陪着老婆一起来看再生育门诊是有必要的。
问：如果第一胎是剖宫产，第二胎有可能顺产吗？
游一平：有统计数字表明，目前我国剖宫产率接近50%。如果剖宫产后再次怀孕、分娩，产妇所面临的风险确实比正常阴道分娩后要高。我认为，第一胎是剖宫产的话，第二胎还是有机会顺产的，前提是第二胎有没有上次剖宫产的指征，比如胎儿宫内窘迫，子宫收缩乏力，胎位不正等情况。
如果想要不再剖宫产，准妈妈在怀上第二个后要积极进行产检，请医生关注子宫和胎儿状况，一有出血、腹痛等情况要立即治疗。
问：如果平时采取了避孕措施的，想生二胎该特别注意什么？
李雪英(省妇幼综合治疗科主任医师)：如果是口服长效避孕药，停药后可能有一定蓄积，至少停药6个月以上再妊娠。而口服短效避孕药的情况稍微复杂，比如复方口服避孕药0号、复方口服避孕药1号、复方口服避孕药2号、复方左炔诺孕酮、复方左炔诺孕酮三相片等一、二代产品，最好也先停药6个月；如果是复方去氧孕烯(妈富隆)、复方孕二烯酮(敏定偶)、复方醋酸环丙孕酮(达英—35)，屈螺酮炔雌醇片(优思明)等三、四代产品，主要成分是孕激素和雌激素，一般认为还是相当安全的，停用次月即可怀孕。
一般来说，取环后生殖器无感染现象、月经规律，就可以怀孕。但如果有计划怀孕，最好提前3个月将避孕环取出。因为宫腔内放置的避孕环可使子宫内膜出现无菌性炎症反应，白细胞和巨噬细胞增生，子宫颈液发生改变，可以破坏胚激肽，使受精困难；另外铜制的避孕环还可以释放铜，不利于受精卵着床，还影响胚囊的发育，这对于胚胎或胎儿的生长发育不利。同时，在取环后3个月(2—3次正常月经周期)再受孕，以便给予子宫内膜一个恢复时间，利于优生。在过渡时期最好选用避孕套避孕。
